Callback requests
If you use an enabled callback form, it asks for your own name, email, optional phone number, relationship, service interest and preferred reply method. Your consent allows Guardian to contact you about the request.
The request is sent by email to the agency. The theme does not create a saved submission record in WordPress. Email systems, hosting services or separately installed plugins may keep records or logs.
Keep sensitive information out of public forms
Do not send diagnoses, insurance identifiers, birth dates, assessments or other client records through a callback request or general email. Ask Guardian to arrange the designated intake process.
Other website features
The theme does not include advertising trackers or analytics. WordPress, your browser, hosting and any additional plugins may use cookies or logs. External links, including maps and Minnesota DHS resources, are operated under their own practices.
