Individualized home supports
Build skills.
Build your confidence.
IHS with training focuses on purposeful practice and skill-building for adults. Your support plan connects the work to your assessed needs.
Practice with a purpose
Your goals make it personal.
Maybe you want to feel more comfortable planning your week. Maybe you want a routine that makes mornings easier. Start with something that matters to you, then talk through how to practice it.
Training is the primary focus. Support may also be included as authorized. Service examples on this page are ideas to discuss, not a guarantee of coverage.

From an idea to a useful routine
Everyday examples to talk about.
Your goals will be individual. These examples show how a familiar activity can become a chance to practice; they are not a preset service schedule.
Plan
A meal you choose
Start with a meal you enjoy. Talk through the steps, make a simple list, practice the routine and notice which parts become more familiar.
Prepare
An outing that matters
Think about where you want to go, what you need to bring and which questions you might want to ask. Rehearse the parts that feel unfamiliar.
Organize
A week with more structure
Try a calendar or checklist that makes sense to you. Use it to plan a small number of priorities, then talk about what helped.
A useful training conversation
Try. Reflect. Adjust.
Start by identifying a step that feels manageable. Practice it in a way that fits your learning and communication preferences. Then talk about what was comfortable and what still needs attention.
Success may look different for different people. It could mean needing fewer reminders, making a choice more confidently or finding a strategy you want to keep using.
What would progress look like for you?
- A task feels more familiar.
- You can explain what you need.
- You find a routine you want to use again.
- You know what you want to practice next.
Use your own goals to define progress. These are conversation starters, not promised outcomes.
