  • Do you have a case manager or support coordinator?
  • What days, times or locations would work for you?

Share private records only through the intake process arranged with the agency.

Before services begin

The team will need to confirm availability, discuss fit and coordinate the required authorization and planning steps.

Ask about your rights, how to raise a concern and how to reach the people supporting you.

Prepare for the first conversation

Use the designated referral process for personal records and detailed client information.

What you can ask

  • Are there current openings for this service?
  • What information is needed from the case manager?
  • How are goals and schedules discussed?
  • Where can I review service rights and agency procedures?

You can ask for clarity at any stage.

If an explanation is unfamiliar, ask for it in plain language. If you want a family member or chosen support person involved, discuss that preference. If a proposed next step does not make sense to you, ask what it means before moving forward.
