What is IHS?
Individualized home supports is a Minnesota waiver service with options that focus on support, training or family training. A case manager can help determine which option fits an assessed need.
How do training and family training differ?
IHS with training focuses on adult skill-building. IHS with family training involves a person and/or family in training that supports life in the family home. Eligibility depends on the person’s circumstances.
Do I need a case manager?
A case manager or lead agency coordinates waiver eligibility and service authorization. If you are unsure where to start, ask Guardian about the next conversation to have.
Are there openings right now?
Contact Guardian for current availability. An inquiry does not guarantee a placement, service schedule or start date.
Can my family be involved?
Discuss who you want involved in planning. Family training is a specific service option with its own eligibility requirements.
Can I upload an assessment here?
Please contact Guardian to arrange the designated intake process. Public website callback forms request basic contact details only.
Does IHS include around-the-clock supervision?
IHS has limits and does not cover general watchful supervision or supervision during primary sleeping hours. Ask about appropriate separately authorized services.
Where can I learn about my rights?
Use the Forms & Resources page to access or request agency materials, and ask Guardian how to raise a concern.

What if I am not sure which service to ask for?
Start with a description of what you want to make easier in everyday life. Your case manager can help connect your assessed needs with a service option. Guardian can discuss availability and the referral process.
Can I talk about communication or cultural preferences?
Yes. Bring up how you like information explained, which routines matter to you and who you want involved. Discuss those preferences during the planning conversation.
What should I do if my goals change?
Contact the agency and your case manager to discuss the change. Ask whether your support plan or authorization needs to be reviewed before making a change to services.
How can a case manager refer someone?
Use the Refer a Client button at the top of the site to open Guardian’s referral form. The referral process helps begin the conversation about fit, availability and next steps.
How do I apply to work with Guardian?
Use the Work for Us button at the top of the site. Questions about openings, schedules and role requirements can be discussed through the application process.

What is respite?
Respite provides short-term care while a primary caregiver is away or needs relief. Discuss the setting, schedule and person’s needs with Guardian during intake.
What is ICLS for older adults?
Individual community living supports is for eligible people age 65 and older through Elderly Waiver or Alternative Care. Your case manager can explain the assessment and service-planning process.
Are night supervision and IHS the same service?
No. Night supervision is a separate service for assessed overnight needs. Discuss the appropriate authorization and how services fit together with your case manager.
